Please paste the exact URL of your blog when asking questions here – or link your nickname to your blog as explained in the Sticky 8 Things to Know. We usually need that to give a relevant answer.
Do you mean a header or sidebar page tab that will act as a link to another blog? If so, you just create a page with an html link to the blog as its title. Note that the name of the link must include a space (i.e. two or more words, or one word plus a space), and that in some themes the header tab won’t be ok.

I think I know what the idea is, I’m researching this as well. Once upon a time, I had installed WP Forums and it placed a Forums Page in my pages links (not the regular editable links section). I didnt like the WP Forums and changed the settings to redirect to my old pgpBB2 forums.
Well, I’ve long since deleted WP Forums but somehow the Forums page listing still redirects to my phpBB2 site. I’m trying to change the exact redirection but cannot find any way of altering the page properties. Theres no script whatsoever in the HTML when editing the Forums page.
The current page hangs for a few seconds with a note about redirecting… then forwards you to the other page. Almost like a bit of script.
page link: http://www.homelessdiver.com/blogs/forums/
the path it lands on: http://www.homelessdiver.com/modules.php?name=Forums
I have no idea how I changed it before, must’ve been extra controls in the WP Forums plugin, but I’m trying to change it now and cant find a way, or even where it says its a redirect.

Homelessdiver, your blog is not a wordpress.com blog and your answer is not relevant to wordpress.com blogs where we have no access to php files and cannot use plugins. If you need help you need to ask in the wordpress.org forums.
