Header image not staying

I have tried several times to upload a header image to my blog. It stays for a few minutes and then disappears. I have tried several times to upload my image and it disappears.
I have determined that it’s not a browser cache issue. It’s not an image format issue.
When I check the image link I get a 404 error. The link is listed on my media page but there is nothing there.

I have the same problem…
One picture in the header works on one of my three blogs but not the other two, although they have exactly the same dimensions (1000×288) and saved for web in Photoshop. They disappears from the side after a few minutes.
It turns out that the images has the wrong MIME-Type: “text / html” instead of “image / jpeg” and status message 404 instead for 200.
When I do “save as”, the new name for the image that works is “cropped-” before the file name, the ones that does not works has only the original file name…
