Help deleting error on Contact page

I put my own information on the Contact form when setting up my blog before I realized this form is for Readers to send to ME. How do I delete my erroneous information so the Contact form is blank for Readers?
The blog I need help with is: (visible only to logged in users)

Hi there,
I believe you are seeing is your own information because you are logged into your WordPress.com account; for example, when I click to your “Contact” form, I see my username (Sierra Echo) in the “Name” field, my own personal e-mail, and my blog listed under website followed by an empty comment field.
Essentially, WordPress.com is so efficient that it’s auto-filling that form as you because you’re logged in. =)

Oh! Then I messed it up trying to fix it! Now I have an Oops message and no form. I tried to add another form and now I have two Contact pages (one says Contact and one says Contace Sports Jewelry Studio Contact. I want to delete these two pages so they don’t show up in the menu accross the top as pages to click and also the pages that say Oops so I can re-add only the one correct one. How do I delete the two bad ones and start over? You are terrifice! Vicki

Hi Vicki,
When I click the link to your website, I do not see any links to your “Contact” pages. Did you figure out how to delete them from your WordPress blog? =)
If so, at this point, you’ll want to add a “Contact” page.
How to Add a Contact Form in your Posts or Pages
To add a contact form, go to My Sites and click on Pages → Add. You can also add a Contact Form to existing Posts or Pages by editing them in the Visual Editor.

When I search for my site, only one page comes up. It should have aSports Jewelry Studio page, an About page, and a Contact page. Are you seeing all three when you go to my site?

Hi there,
Your “About” page and your “Contact” page do exist–they are not appearing because you will need to add the links to them to your menu.
Here is a link to everything you need to know about custom menus.
The Cliffnotes:1. Go to your My Sites, and click on Menus to create your first custom menu. **You should already have a default menu set up.
2. Add links to your “About” and “Contact” pages to your menu. (Click the + sign, then “add menu item below”, select the “page” category, then select the page you want to link to, then the blue “add item” button.)
Once you do this, they will appear in your navigation at the top of the page.
