HELP!! Login/Form builder

  I am in the process of designing a summercamp website, and the client prefers for the parents of the campers to submit contact info/ allergy info/etc. via a step-by-step, form-based questionaire on the website. The questionaires will be tied to an individual login so that the user can view/edit the contact info as they go along. Also, the questionaires need to be able to be viewed by an admin account in order to be printed for filing purposes. Does anyone on the forums know of any plugins capable of Doing this type of functionality? Thanks in advance.

  I would use SurveyMonkey or Google Docs for this.
