HELP moving to a self hosted wordpress site?

Hello,
I currently have a blog – lucythechatterbox.wordpress.com
I would like to move this to the domain lucydaniels.co.uk
from what I understand, I would need to use a wordpress self hosted site?
Please could someone help me on what I need to do? I have no idea and I want to be able to keep all my old blog content.
The blog I need help with is: (visible only to logged in users)
