Help with the contact form button

I used the contact form button to add a contact form to my page. On preview it showed MY information in the field INCLUDING my email address and I thought, that can’t be right. Maybe it’s just an example.
When I hit publish, sure enough, the form was on the internet with Kat’s Connections, talktotheshoe.com, and (email visible only to moderators and staff) in each of the fields. Clearly that is not right and I didn’t see a way to change it.
In a rush to delete my email off the web, I changed the post by backspacing over the code for the contact form. Then there was just a blank space. But now there is no code for the contact form when I click the button. I think I inadvertantly deleted the code from the button. All that appears under HTML when I click it is this:
I am not code literate and am brand new to WordPress and blog building. Is there a way to fix my button so I have the contact form again AND it does not populate my information into it?

You’ll need to create the contact for again: http://en.support.wordpress.com/contact-form/
The info that you saw inserted was just a courtesy for you. Whenever you’re logged in as a WordPress.com user, your information is automatically filled in to every WordPress.com contact form. If I were to view your form, I’d see my info there, and if a non-WordPress.com users were to view it, they’d see blank fields.

Ohhhhhh… so I ONLY saw it because I was logged in on the backend.
So, if I had logged out and checked… There would’ve been blank fields.
Well, okay. I guess I panicked for nothing. Ha!
