I would like to know if the upgrade from free to personal includes hosting. Does it include a domain? Does it include an email address and how many. I just know there are always hidden add ons that the budget needs to be tweeked constantly.
Are there hidden extra costs I should anticipate. It is not clear enough on your upgrade comparison towers -

I am also looking at the Bluehost offer. But not sure as with Blue Host it works out at $59 per month. What extras are they giving in comparison to going with WordPress direct? -

All WordPress.com plans include hosting. The Personal plan does include one custom domain name of your choice.
Does it include an email address and how many?
It does not include an email address but you can however set up email forwarding with your domain name:

I am also looking at the Bluehost offer.
If you wanted to go with Bluehost, you would be setting up a self hosted site, not a WordPress.com site. If you are not familiar with the difference between WordPress.com and WordPress.org then I recommend you to give this a quick read:
