The price you are seeing on the plans page is the monthly price. For example, for WordPress.com Premium the monthly price is $8. Billed anually that comes out to $96 per year.
All plans come with a 30 day refund policy (for everything except any domain names added). This allows you to try out a plan to make sure the features are right for you.

If you want the Premium plan and to be able to monetize your site, yes, $96. You can always start your site on the free plan and get things set up and then later upgrade to Premium.

Yes, sites on our free plan will display ads to help WordPress.com generate revenue to pay for the costs associated with your site.
