  I started what I thought was a free trial for the domain outsideinnutrition.com. It was showing as outsideinnutrition.wordpress.com, so I went ahead and paid the $99 fee to upgrade to get the domain name as a .com site (which I thought I was registering for in the first place). It is still showing as outsideinnutrition.wordpress.com. Do I need to get a refund and start over?

  No need to start over. :)

  Just visit your domain settings here:

  https://wordpress.com/my-domains/

  Hit the dot next to “outsideinnutrition.com ” and then click Update Primary Domain.
