How Can I Add A Contact Form For My Site?

The question says it all really, I just want to know how I can add the contact form onto the “Contact Us” section of my site…

One problem I am having with the contact form is it lists my name email and site name on the contact form when I posted it onto my website when I haven’t filled that in so it should be blank and ready for users to fill in. Please can you help me with this issue thanks.

Also note there is an Admin [edit] link that is only available to you when you are logged in. No one else can see it or use it.

So basically the contact form which is currently displaying my information even though i left that blank will not be on there anyway once I have signed out of wordpress or another user goes onto the site? They will just see a blank contact form only is this correct?

To test this log-out of WordPress.com, clear your browser cache and cookies, then visit the blog as if you are a visitor ie. without logging in and check it out.

I am trying to add a contact form to a new page. I am following the directions, but the published page does not show a form at all. Any ideas?
