How can I add my pdf Enrollment Forms to my site for parents to click on, fill out and print out.

    I want to LIST my ENROLLMENT FORMS separately that I have saved in pdf format, for each parent to click on and fill out. I want to say this:
    Parents; you may print out the following Enrollment Forms and fill them out by hand OR fill them out in ‘word’ (using Acrobat or Reader choosing Tools and Fill & Sign) and then print them out; or choose to pick up an enrollment packet in person at the center between either 8am-11am OR 3pm-5pm. Then, after your child has been accepted for enrollment, you can e-mail the documents ((email visible only to moderators and staff)) OR deliver them in person on or before your child’s first day.

    On Your Child’s First Day of Preschool,
    You will need to bring…
    *Extra set of clothes to be left in cubby (in case of an accident)
    *Small, soft doll or toy to fit in cubby (to use at nap—blankets are provided)
    *Required Forms on or before first day
    a) Immunization Record “Physician’s Report”
    b) Completed “Emergency Contact Information” form
    c) Tuition (check, cash or direct deposit via Facebook) for the first month
