How can I get rid of my information in the contact form I created?

every time I create a form, when i post it, it automatically fills the form information with my information (making it public to all) ex: Name: fills in my name. Email: fills in my email, Website: fills in my primary blog web address. Any idea how I can fix this??

It’s only filling it in for you based on your WordPress.com account as a courtesy.
Other logged in WordPress.com users will see their information automatically filled in, and non-users will see a blank form.
