How can I move my followers from a WordPress.com site to a self-hosted site?

I’d like to move my followers from on3designsdotcom.wordpress.com to http://www.on3designs.com. I’ve read on forums that you can help with that. Any insight on how to get this process started? The blogs are exactly the same, just updated look and feel, with more customization.

Here are the instructions! Moving your subscribers
- On your self-hosted WordPress site, install the Jetpack plugin, connect it to WordPress.com with your WordPress.com username, and make sure the Subscriptions module is activated.
- Optionally, add the Jetpack Subscriptions widget to your sidebar.
- Optionally, on your WordPress.com site from which you’ll want to transfer your followers, post a notice telling your site’s followers that you’ll be moving, and that their subscriptions will be going along with you. This is because, while your email subscribers will continue to receive email notifications of new posts as before, WordPress.com followers will only see new posts in the Reader. They will not receive email updates unless they subscribe to receive those on your new site, which they can do if you’ve added the Subscriptions widget.
- Once your Jetpack is connected and ready to go, and you’ve informed your readers about your upcoming move, contact us and we’ll move your subscribers for you.

Your WordPress.com subscribers and email followers have been transferred as requested.
Please note that any WordPress.com followers who opted to receive email updates will need to re-subscribe again on your new site through the Jetpack signup box, otherwise they’ll only see new posts in their Reader. Non-WordPress.com email subscribers will continue to get email updates.
You might think about setting up a Site Redirect for your WordPress.com site at on3designsdotcom.wordpress.com to send your site visitors to your new domain. Otherwise, people will still be able to subscribe on your old site.

I am evaluating to update to Premium but I still have some doubts, one of them related to this topic.
if I buy the Premium upgrade, the re-direction of all my followers to the new site is a service included in the update or not?
Are the posts already published (in my case in photosatriani.wordpress.com transferred to photosatriani.com?
