How can I prevent my email address appearing in my contact form

When I insert a contact form on my blog it automatically fills the fields – Name and e Mail address – with my name and email address! How can I make sure it shows up as blank?
The blog I need help with is: (visible only to logged in users)

Your name and email address are visible only to you not to the public. Your blog visitors will have to put their own.

If you want to see an empty form – try logging out and looking at the contact form –
if you are logged in WordPress.COM fills the form part way out – then when the form is received it wall also have a part to the effect “sent by a verified WordPress.COM user”

Note WordPress.com uses cookies to keep track of commenters so if someone makes a comment on your blog, the next time they come to your blog, unless they have cleared their cookies, WordPress.com will recognize them and complete the details for them. No one else will be able to view those details in comment boxes or in contact forms. The next person who comments will see blank fields. Commenters will not see each others details.
