How do I add real estate forms to my blog?

  How do I add real estate forms to my blog? ie mortgage calculator, buyers and sellers forms for clients to fill out and it goes directly to my email? I tried using the contact form and just adding fields, but it would only let me add one original field.

  The blog I need help with is: (visible only to logged in users)

  There are code restrictions on free hosted WordPress.com blogs for security reasons and among them are form codes, JavaScript codes and Flash codes. http://en.support.wordpress.com/code/
