How do I create a contact form?

  I would like to have a private, anonymous form on my site where readers can submit questions, content ideas, etc, without having to identify who they are or go to another site (such as formspring) to submit it. Any information would be greatly appreciated.

  The blog I need help with is: (visible only to logged in users)
