How do I edit a page for Table of Contents?

Please see http://specialasianfood.com and click View Menu Page of The Oriental Cookbook and you will see The Table of Contents. Anyone can show me how to draw the horizontal and vertical lines with heading Nos. Table of Contents and Pages will be appreciated.
