How do I fill out a support ticket to transfer subscribers from wordpress.com?

Transfer subscribers from wordpress.com to wordpress.org. I read in these support docs that I have to fill out a ticket to support in order for them to transfer my subscribers from my wordpress.com account to my wordpress.org blog. However, I see no support ticket to fill out.
The blog I need help with is: (visible only to logged in users)

Staff is away for a couple of days, but they monitor the forums and will see the modlook tag on this topic. You can subscribe to be notified of a response.
Please provide links to both your WordPress.com and WordPress.org sites, and be sure you have JetPack installed.

I’m happy to transfer the users who have subscribed to your blog either by email or via the WordPress.com Reader.
Please keep in mind that while your email subscribers will continue to receive email notifications of new posts as before, WordPress.com followers will only see new posts in the Reader. They will not receive email updates unless they subscribe to receive those on your new site. You might want to post a notice on your old site about your upcoming move to keep your readers informed.
Can you please reply back if you have any questions or let me know you would like to go ahead and move your subscribers?
