How do I get the contact form to be blank boxes?

  I added a “contact form” to my “Contact Me” page. Instead of it having blank boxes–it automatically has my name, email, and website entered. How do I get the contact form blank so visitors of my website can fill out the information and not see my personal information?

  They’re generally blank, but they automatically fill in information for any logged in user.

  So, you’ll see your information, I’ll see my information, and visitors who are not logged in will see blank boxes.
