How do I insert an Anonymous Contact Button on my Blog?

Hey WordPress Fam,
I’d like to be able to accept anonymous prayer requests from readers of my blog. I want a quick submit form on the side of my blog, but am not sure how to transform this idea into reality.
Any insight would be most appreciated.

Create a page http://en.support.wordpress.com/pages/
Customize a contact form to appear on it http://en.support.wordpress.com/contact-form/ and embed it. -
This is going to sound ridiculous, but what does “embed it” mean? (I am a beginner blogger.)

Please just create the page and use the instructions for creating a contact form on the page. Before you start I recommend reading the instructions as reading them first always helps me.

Follow the instructions above and it will embed itself –
NOTE: Edit the form to not require the name and other fields to be filled out – I think default is to require a name and email
