How do I make a form, that clients can upload their photos?

I am making a form and would like people to able to upload their own photos. They would file out the form and then at the bottom there would be an “upload Photo” button.
The form and all the information, plus the photo would be emailed to me. And then I can size, filter and post the photo and content to my blog.
I really hope this exists and is easy.The blog I need help with is: (visible only to logged in users)

That’s not possible on a WordPress.com blog. This is the WordPress.COM support forum. You need to hire a web host http://wordpress.org/hosting/ and set up your own WordPress.ORG install for that.
WordPress.com and WordPress.org are completely separate and have different logins, features and run different versions of some themes with the same names. Read the comparison here http://support.wordpress.com/com-vs-org/
If you don’t have a username account at WordPress.ORG click http://wordpress.org/support/ and register one on the top right hand corner of the page that opens, so you can post to the support forums there and receive advice from WordPress.ORG bloggers.

If you need the upload option then you need to look at a WordPress.ORG install as noted above
If you stay at WordPress.COM a picture can not be attached to a form – visitors would need to upload pictures to a file sharing service and put the link in the form – then you would need to fetch the picture yourself

If you want to embed the document itself into a post, then you will have to use a third party application like Scribd or Google Docs.

I’m trying to make the “upload photo” super easy. The idea: a person finds a free piece of artwork. On the back of the artwork it tells them to take a photo and upload it to “us”. We then will share the free artwork and details of the art-find. I can imagine someone with their smart phone, finding some artwork and clicking a photo and uploading on the spot. Making it easy, will make it happen.
