How do I make an index?

  Hi,

  I quite like how towerofthehand.com does chapter by chapter summaries and am wanting to implement a similar style on my site.
  I’m unsure of how they make a page for the book and then make the chapter pages which are then linked to that one book. Also how they get a table with three across rather than one.

  I think having one page/post per chapter is alot easier to consume then having one page which contains around 20+ chapters.
