How do I move my followers to my self-hosted WordPress blog?

Tomorrow, April 11, is my 6 year blogging anniversary with WordPress. This year I moved my blog to a new domain name that I purchased myself and installed the JetPack plugin. I resumed blogging, but now I get almost no traffic, few comments, few likes. I think I need to import my followers from my old location. How is this done?
Old url: http://jimmycrackedcorn.wordpress.com
New url: http://garlicandhoney.comThere are a lot of people who were following, and I don’t think they’re seeing notifications of my new posts.

I’m using the word Followers, but I might mean subscribers, or I might mean both. At any rate, thank you for your help.
