How do I remove a site from my list of blogs?

On the “My Blogs” list for my WordPress.com there’s a blog listed there that doesn’t exist anymore:

Incidentally, my site is showing up on someone else’s list of “My Blogs” who is not a user on my site. We can’t figure out why this is happening, but I would like for my site to be deleted from his list (and anyone else’s it might be hanging out on)–I submitted a security issue on that, but I haven’t heard back from anyone about it yet. How can he remove it, and is there a way that you can see if my blog is mysteriously popping up on anyone else’s account? I only have one user on my self-hosted site, so it’s really flummoxing that someone else’s account got linked to my personal, self-hosted site through WordPress.com. Here’s my site URL:

In reply to the automated message, yes, I know that it’s not a hosted account but it’s showing up on my WordPress.com list of My Blogs and I can’t get rid of it.
Also, the other issue is of some concern as well.

I have the same issues.
I have some old self hosted blogs on my wordpress.com account.
I want to clean up my wordpress.com account by removing these old accounts. The old self hosted accounts have been deleted a long time ago but still show up on my wordpress.com account.
I do know it has something to do with using the wordpress.com plugin on self host accounts.
But for the love of God! I can’t remove them from wordpress.com account!
I see this posted everywhere and no answer! WordPress will say post this on wordpress.org and wordpress.org will tell you to post this on wordpress.com
Endless loop of irritation! Maybe someone will share a fix?

Needless to say I’m concerned that WordPress.com is giving other users access to my self-hosted site (or any site for that matter), and I want to sort out why that happened and also have my site removed from anyone else’s list of blogs who is not me.

There are no users connected to that site anymore, only you were the one that was connected.

Okay, just emailed with the person who had it attached to his account and it looks like he had support remove it on a separate ticket this morning.
I still don’t understand how he got access to my site in the first place. How is it possible that someone who isn’t a user on my self-hosted site could end up with access to my site on their WordPress.com list of blogs? Removing it is great, but understanding how and why it happened in the first place would be better for everyone.

The other user was connected to alexisclements.com about 4 weeks ago. It’s possible that he/she did got access to your self-hosted site. Please let us know if you need any further assistance!

I don’t know. We’re only able to control accounts and blogs at WordPress.com. Best might be to change the password of your self-hosted account to prevent this from happening again.
To clear up any confusion, WordPress.com and WordPress.org are two different entities.
