How do I remove an old self-hosted site from wordpress.com/sites?

  My self-hosted WordPress site (connected with Jetpack) no longer exists, and I’d like to remove it from wordpress.com/sites. I looked at http://en.support.wordpress.com/disconnect-jetpack-sites/, but the instructions don’t match what I see in the UI.

  The blog I need help with is: (visible only to logged in users)
