In order to delete any blog one must be logged in under the exact username account that registered the blog. Provided you are logged in under the username account that registered the blog you can delete it by using these instructions. http://en.support.wordpress.com/delete-site/
Please note that deleted blog URLs are not recycled.

Since the blog is related to your name – it looks like a personal services blog – I vote with @TT above to just set it Private if you no longer need it – then it will be waiting for you if you need a blog for any activity in the future – we have several people each day show up and “I deleted a blog with a good name last year can’t I please have it back I have changed my mind” – and the answer is always the same – gone is gone forever
