How do I transfer followers from hosted to self-hosted?

  Hello I am trying to import my followers/subscribers to my self-hosted blog, from my previously hosted WordPress blog.

  I have Jetpack installed and connected with WordPress.com, even with that on the WordPress app it is still telling me to install the plugin.

  How do I transfer them over?

  The blog I need help with is: (visible only to logged in users)
