how do you remove your details from a feedback form?

i can’t remove my name and email details from the feedback table. When you log in from the outside they are stil showing.

If you are asking about the contact form, then you see your name and email details while you are logged in to WordPress.com. Other logged in wp.com users will see their own names and email addys. Not logged in folks won’t see either a name or an email address.

it’s called a ‘custom form’…is that the same thing?
A friend logged in via the main url and the details were still showing, even after sending them through to the relevant email.
basically the table is being used for people to send details through if they are interested in finding out more…i’m not convinced all of them will notice that they need to delete the existing details and replace with their own. If this happens, we miss out on the interest of prospective parents.

on the page titled ‘school support survey’
sroll down and the table is underneath the original text (name/email/postcode/d.o.b/plus a question with a drop down menu)

OK, there is something wrong: I get a page not found error. Is the page actually published? (I’m a volunteer and cannot view unpublished pages.)

I can see it now. The form looks and acts as it should: I’m logged in on Firefox and the form fills in my public name and my email address. I’m logged out on Safari and there is no name, no email.
Try logging out from WordPress.com and use a different email address when testing.
