how easy is it to design your own WordPress layout?

  I have been using the template layouts at WordPress for years if not decades now and in my mind I have a great layout which I cant seem to find on any in the library.

  Is there such a thing as design your own and if so how do you do it?

  I wonder if WordPress or any of the templates is capable of amending the layout and content to provide something more like this rather than just a stream of blog posts!
