How to add a contact form to my blog?

  I would like to add a contact form to my blog, however I cannot find one on WordPress that can easily be installed.

  I found one on WordPress.org that instructed me to up load it through the plugins option, however I do not find that option in my dashboard.

  This form is very important in blogs and it should be simple to install, like a widget.

  It is becoming somewhat complicated and frustrating.

  I would appreciate your help.
