How to add an 'Ask Thadd' widget for visitors of page?

Greetings….I’m trying to add an ‘Ask Thadd’ widget on my page that would allow visitors to my blog to ask me questions that would be emailed directly. I searched through widgets and couldn’t find out how to do this. I saw this feature on someone else’s wordpress blog and would like to add to mine. Any information or assistance is appreciated. Thanks!

They were possibly using the contact form shortcode in a text widget. Do you have a link to the WordPress.com blog where you saw the example you want to replicate in action?
