How to bring back the "contact form"

    Hello, Im not sure what I did but the contact form with separate “name”, “email”, “site” and “comment” dissapeared… I would like to have it on my site – how can I bring it back?

    I am here today because I have this problem.
    I posted my new blog for my site Bookseedstudio & the comment link/button/info is gone. No way to comment. And last month it was here. And for at least 10 years previous.

    I don’t remember adding any plug ins or doing anything new or fancy.
