how to change title on one page

    I have created a blog for a client. There is a page for the blog, plus other pages on the site ( http://s98912242.onlinehome.us/ ). As you see the header title is the same on every page. I would like to change the header title on the blog page to Blog: Family Writing Project. I am using theme Twenty Twelve which I have modified.

    Does anyone know how I can do this please?
    My blog is below, that is not the one I am having issues with. Please use the one listed above. Thank you.
