How to clear contact form of my own address?

  My contact form has my personal contact information in it. Whenever you navigate to that page, my email shows up in the email boy and my full name in the name box. How do I keep this from happening?

  It only shows up that way to you. Anyone else who goes to the form will either find it empty, or with their own information in it.
