How to create a form like this

Hello I want to create an form of showing some products with different packages and anyone knows how can I add this to my current website, is there any plugin to do that or anyone knows how to add that without installing any other template that has this function of forms already?

From what it looks like, it’s a pricing table. You can create pricing tables using custom code, plugins or using templates that support this feature.
I searched for a few on the WordPress plugins repository, and it looks like we have a few here. https://wordpress.org/plugins/search/pricing/
