How to declare WordAds revenue on taxes?

    Hello, I have a question:

    Is there an official 1098 form (United States tax code) that I can use to declare my earnings for revenue I received using WordAds? If not, is it acceptable to just declare my earnings from 2012 on my taxes? This is the first time I’ve tried to include online revenue on my taxes, so I’m not quite sure how to go about doing this properly.
