How to fix broken WordPress form?

    I’m currently having issues with all of the different contact fields working at once. Currently, 4 parts of the form are showing up as plain text. I have copied and pasted what I have created using the editor below:

    I’ve gone through everything to my knowledge, is there something I’m missing? Or is this just too much for the webpage to handle?
