You can only follow them from your account here if they have the jetpack plugin installed and activated on the self-hosted site. If they do not, then you cannot follow them.

Another way to answer the question is… use a feed reader, such as Google Reader… and subscribe to them. They won’t show up here on WordPress.com among the blogs you follow, but you will see every post.

Just to be clear as many bloggers are confused WordPress.com and WordPress.org are completely separate blogging communities.
