How to input a form onto a page in theme called Forefront?

I’m going to set up a new wordpress site, ideally using the theme ‘Forefront’. I’m really stuck because I don’t know whether this theme will allow me to include a form on one of the pages – ie rows and columns where readers can input their details (name, location, pledge) and also submit their email (but only I can see that.) Secondly, I don’t understand how to create this form and don’t have the budget for a pro to build it for me. Someone suggested a plugin but I don’t know how to install that onto a WordPress page. Any thoughts very appreciated.

The only form we can use on any free hosted WordPress.com blog is this one > http://en.support.wordpress.com/contact-form/

If you require any plugins you will need to hire a web host http://wordpress.org/hosting/ and set you your own WordPress.org software install to install them.
There is no FTP access and no blogger installed plugin capability on free hosted WordPress.com blogs.
