How to integrate my e-mail form into wordpress.com (not .org)

  I received a Benchmark-Plugin for download to integrate in my wordpress blog, unfortunately I don’t know where to add this new element. The target is, to integrate subscribers from my blog directly into the address list of Benchmark E-Mail marketing. Thanks for help.

  But how can I integrate an e-mail form which leads to the site of Benchmark or a new one with follow-up option? The normal “follow this blog” is already integrated via widgets.
