How to make a “tip us” or “Feedback” page

  How does one make a page or a button to lead to page where it has slots to enter your name, email address, and a box to enter your feedback or tip..

  Make sites have one such as Engadget.com “tip us on news”

  I would like to make a page like this

  Can anybody help?

  Sure. Just do a search of the FAQ for “Contact Form” and you’ll find it.
