How to remove a Jetpack blog from my wordpress.com account

  Someone used my wordpress.com account to connect jetpack to a site in Brazil. I’ve changed my password, but I’m not sure how to remove the site from the list of “My Blogs”. It’s this site: omniwp.com.br

  The blog I need help with is: (visible only to logged in users)
