How to remove my default contact data from my Contact Forms?

How do I remove my own contact data from my Contact Forms? It’s going to confuse my readers, who need blank spaces to type in their own contacta data.

You can see here how the contact form at the end of the article comes pre-loaded with my own contact data. I would prefer to have blank spaces there so my visitors can type in their own data. Otherwise, if they’re obliged to erase my data before typing in their own, they’re apt to get confused.

The contact form is automatically filled in if you are logged into WordPress.com. Your visitors won’t see this unless they are logged in to and if so they will only see their own data.
