Hi there, to confirm, are you referring to http://cosmocaredental.wordpress.com/? All WordPress.com domains come with SSL but only sites with the Business Plan can install custom plugins.

yes, I’m referring to http://cosmocaredental.wordpress.com/? , I bought the custom theme, Just want to know how to set up a book an appointment with date & time.

Hi,
are you talking about the “Appointment Booking Calendar” (APB) plugin by author “codepeople”?
If so, and you already installed the plugin, to set up the calendar, in your site admin page click on “Plugins”, and then “Appointment Booking Calendar”, which will take you to APM settings page.
You’ll find there one calendar already created ( a “Calendar Item 1” ) – you can change its settings by clicking on “Manage settings”.
Adding calendar to your page, copy the shortcode (for example, [CPABC_APPOINTMENT_CALENDAR calendar=”1″] ) into any page (or post) you created.Hope this will help.

By the way, sadly, only sites with the Business Plan can upload custom plugins. However, all sites do get access to some plugins automatically.

@torres126 is correct when they say custom themes and plugins can only be used on WordPress.com sites with the Business plan — cosmocaredental.wordpress.com is currently using the Free plan so there won’t be an option to use the theme until you have upgraded.
