I am going to create a site for a client, but he wants to keep his domaine name, he already have an old web site, so how can i do it with wordpress? Do i need to create another account? i would like to have somebody by phone, is it possible?

You can create a new website with the same domain name…Just transfer all the data from the old website to WordPress…Don’t forget to create a backup of the old website data before transferring it to WordPress…

You can create a new WordPress.com site on this page.
You can choose a free site address like sitename.wordpress.com and when you want to link the custom domain that your client is already using, you will have to use the domain mapping feature to link that domain. Learn about it in detail here.
It is possible to import your blog content from a variety of other blogging platforms, including Medium, Blogger, Israblog, Movable Type, LiveJournal, Tumblr, Typepad, Xanga and more. Learn how to do that here.

There are a couple of options for using an existing domain on WordPress.com:
1. Connect the domain to the WordPress.com site with domain mapping:
https://en.support.wordpress.com/domains/map-existing-domain/
2. Transfer the domain to WordPress.com:
https://en.support.wordpress.com/incoming-domain-transfer/
Do you want to import the content from their existing site or start afresh?
Do i need to create another account?
This depends on how you wish to manage the site. It can be created in your michellegravina03 account and ownership can be transferred to your client’s account at a later stage. Doing so will mean they have ownership of the site and can handle renewal payments for upgrades.
