How to transfer followers on a wordpress.com hosted blog to self-hosted install?

I’ll be moving a client’s blog from wordpress.com to a self-hosted installatino soon. She was concerned about migrating her followers along with the content, but I don’t see anything tht tells me how to do this. She does not yet have her new hosting service but will in a couple of days, so I’m giving you her wordpress.com url here: http://paperandbooks.wordpress.com/

Strike that hannahwestdesign.com sentence! It has nothing to do with my question, I just haven’t connected to her blog yet.

1. You install the JetPack plugin and activate it on the self hosted install.
2. You post the .wordpress.com URL and the self hosted WordPress.corg site URL into a thread like this on and request the tranfer.
Staff then transfer the email subscribers only as those using the Reader cannot be transferred. -
