How to used Contact Form 7 ,

Good afternoon,
I want to know if I can upload module ¨Contact Form 7¨ in my blog, in condition in which blog used WordPress servers, and basicaslly I dont have access to my folders from site, where I can put this additional free pacckage.
Aniway, if somebody can help me with some sugestion in which I can made my own formular to subscribe and loggin or import one from some other surce in some form recognise by WP, please help me. The formular must be very simply and must contain name, forname, username, email,capcha. The IP from subscribers will appear in my email mannager from WP.Thanks in advance for any advice, support or ideea.

Thank you very much for indication. Unfortunately I see that is not enough for me, because on their pre setup is not included also the pass option.
On the other side, must be recognised by the server email and site. If the user will subscribe, then when will logged must used the ¨username¨ and ¨password¨ and after that must have access to my site. Here is my problem. I see that all widget or other options is available only If I have my own server. I use WP server.
How I can added this additional package ? This is my VERY BIG TROUBLE in this moment.Thank you for support. I waiting suggestions or ideas.

There is no way for you to upload anything to a wordpress.com site.
There seems to be something of a language barrier, but since you’re asking about Usernames and Passwords, so you may be asking about making your site Private:
