Html code isn't producing the form intended

I’m using mailchimp and am trying to add the sign-up form to my site, through both a sidebar widget and as part of blog posts and pages. Every time I post it, only the code itself shows in the live site. Occasionally it will show correctly in the preview (for the page or post but not for the sidebar). Here is the code:

The code is not working correctly because it uses a “form” HTML tag that is not allowed on WordPress.com for security reasons as explained here:
