HTML coding help required for Form

  I want to add a custom form my website.
  Can you help me with the html coding required for designing a custom form with the following fields.

  Title : SHARE YOUR RECIPE
  Recipe Name ———————–
  Description —————————
  Dish type ————————–( a drop down menu required)
  Difficulty level ______________
  No of serving ———————
  Preparation time
  Cooking Time
  Ingredients
  Directions/Method
  Name ——first name ——–last name———–
  Email————————————–
  Submit (button) By clicking the submit button you agree to my terms & conditions. (terms & conditions to be hyperlinked)

  I did try and embed the form which was made using Jotform but it did not work. don’t know why!

  The blog I need help with is: (visible only to logged in users)

  Hi! Jotform likely has some code we don’t allow here:

  Add Code to Your Site

  But you’d be welcome to create a form using the Form Block:
  https://wordpress.com/support/wordpress-editor/blocks/form-block/

  Basically you’ll just add the form, then add the fields you want. Ingredients would need to be a text area, etc.
