I added a contact form but when I published it, my info was already in the field

  I added a contact form but when I published it, my info (name, e-mail address, etc) was already in the fields. How do I remove it?

  The blog I need help with is: (visible only to logged in users)

  It’s auto-fill. If I go to your contact form, it automatically fills in my information. Try logging out to see that your contact form will no longer have your info visible.
